Description.
The Institute of Medicine (IOM) (2011) states that health professionals’ primary role for pain conditions should be guiding, coaching, and assisting patients with day-to-day self-management to reduce the risk factors that drive failed treatment, delayed recovery and chronic pain. Transformative care is a patient-centered model of care including both self-management training and health coaching with treatment to improve patient engagement and long-term outcomes. However, there are many barriers for health professionals to implement transformative care routinely in clinical practice including the lack of reimbursement, time burden, and inadequate training skills coupled with the lack of care coordination, fragmented care, poor communication, and conflicting treatments. This webinar reviews both the health professional and patient experiences in implementing transformative care.
Provider experience includes evaluation, reviewing diagnosis and causes of chronic pain and the pain cycle, treatment planning to introduce transformative care with telehealth coaching, shifting to patient-centered clinical paradigms, and follow-up with patients to reinforce self-care. The patient experiences include acknowledging their role in successful outcomes, completing pain and risk assessments to assess their pain, risk and protective factors in all areas of their life and readiness to change. Patients then undergo on-line training to implement action plans including a positive mind-set and healthy body, lifestyle, emotions, spirit, social life, and environment. The patient is supported by biweekly telehealth coaching visits to provide support to them in making the lifestyle changes needed to heal and recover from pain conditions. The patient’s progress in both engagement and improvement in pain and interference is monitored to ensure success.
Objectives:
1. Review the health professional experience in implementing transformative care including patient evaluation, treatment planning, shared decision-making, and shifting to patient-centered clinical paradigms.
2. Explore the patient's experience in implementing transformative care including pain and risk assessments, telehealth coaching, on-line training to reduce risk factors in each realm of their life, and implementing action plans each day to relieve pain.
3. Examine steps in following up with patients by the health coach and health professional to ensure success of transformative care.

The series will focus on different aspects of patient-centered transformative care for pain conditions. They will introduce telehealth coaching as part of the care team for pain conditions to prevent chronic pain and addiction in routine healthcare.
Each webinar will also introduce the full clinical course for health coaches to provide the background, rationale, implementation and patient and provider experiences of transformative care for pain conditions. The longer 14 CEU course will offer an in-depth look on various aspects of these topics and is designed for health coaching and health professionals to collaborate and be reimbursed for transformative care. Webinar February 24, 2022 will be facilitated by Dr James Fricton
Dr. James Fricton is a Professor Emeritus at the University of Minnesota, Pain Specialist at Minnesota Head and Neck Pain Clinic, and current President of International Myopain Society. He is trained as an Anesthesiology Pain Management specialist from UCLA School of Medicine. He has 40 years of National Institutes of Health sponsored research, University-based education, and patient care of chronic pain patients. His research has developed the concepts and tools to implement transformative care to prevent chronic pain as part of his national campaign for preventing chronic pain and addiction with International Myopain Society (www.myopain.org).

Emily Ness, MA, NBC-HWC is a Nationally Board-Certified Health and Wellness Coach with an M.A. in Integrative Health and Wellbeing Coaching from the University of Minnesota. Emily has coached as an intern at the VA hospital, working with patients diagnosed with Type II Diabetes and at Catholic Charities, providing coaching services and yoga to employees. For the last two years, Emily has been working with the PACT program, coaching patients with chronic pain. Her background Includes Health Coaching for Chronic Conditions with additional training or certifications in Yoga teaching, Ayurveda, Lifestyle Medicine, Functional Nutrition, Reiki healing, guided meditation, transformational coaching, narrative work, identity and archetype exploration, and group health and wellness coaching.

Megan Hadley, MA, NBC-HWC is a Nationally Board-Certified Health and Wellness Coach with a graduate degree in Integrative Health and Wellbeing Coaching with a Minor in Health Equity and expanding healing practices from the University of Minnesota. Her background also includes administrative coordination of a multidisciplinary clinical program. She is currently coaching patients with pain conditions using the PACT platform as well as in her private practice, Harvest Health and Wellbeing. She has additional training and experience in Reiki healing, nature-based approaches to healing, guided imagery and meditation, storytelling and archetype work, and community-based participatory research. Megan lives her vision through building community, reconnecting with and honoring nature, and continuing to consider how health coaching can build health equity and a more just and healthy future for all.

Michele Rae is a transformational coach who is passionate about supporting clients living an empowered and authentic personal and professional life. She is an NBC-HWC (National Board-Certified Health & Wellness Coach), spiritual director and has an MA in Human Development.
